We talked with a plastic surgeon about unsuccessful plastic surgeries, their consequences, and possible correction options. Spoiler: science and the industry of aesthetic medicine do not stand still, and today almost all cases of unsuccessful operations can be corrected.
Let us define what we mean by unsuccessful operations. Aesthetics is a subjective value, and the boundaries of the “unfortunate event” concept are very blurred. First of all, you need to understand what the patient wants. That is, the surgical plan must be built towards the patient, to his wishes, if this, of course, does not contradict the surgical laws or the specifics of the anatomy in a particular case. But let's talk about the most popular plastic surgery in Russia today: nose surgery, breast surgery, and eyelid correction.
Rhinoplasty
Rhinoplasty is one of the most difficult operations. Statistically, from the point of view of aesthetics, unsuccessful operations are most of all in the field of rhinoplasty (according to various sources, it is even about 15-20%). After all, nose surgery has a number of features. First of all, it is work with cartilaginous and bone structures, while cartilage has a shape memory. That is, if your nose was quite twisted, during the operation the surgeon tries to build the entire cartilage and bone structure. This is a fairly complex relationship and complex technical process. But even having built this structure correctly, nobody will be able to guarantee that the cartilage will not strive to occupy its previous position. Now the trends are such that rhinoplasty becomes completely organ-saving. Aggressive operations, when cartilaginous structures were cut out, removed all the "excess" in order to make the noses small and fragile, disappearing into the past. Advanced plastic surgeons in the European and American rhinoplasty communities promote a new approach. The tendency is related to the fact that the effects of operations of the previous format began to manifest themselves: gross changes in the shape of the nose with the passage of time can cause difficulties with nasal breathing up to its complete difficulty. And the shape of the nose-pipette is no longer in vogue.
Mammoplasty
Mammoplasty is probably the most frequent operation today, so there are more complications than in the vast majority of plastic surgeries. When working with the breast, it is important to take into account many aspects: the shape of the chest, the length of the chest, the height of the person, the weight of the body, the shape of the breast, which is completely different - wide, narrow, conical. Therefore, breast surgery is not as simple as it might seem to ordinary people and most patients who simply don’t want to understand that the body is a much more complex structure than the LEGO designer. It so happens that surgeons make a mistake, as a result of which the operation can be considered aesthetically unsatisfactory. It would seem that the doctor did everything right, he walked the path, but this time he did it differently. The situation suggests that each patient is unique and the surgeon’s skill is precisely in the ability to see these nuances. It is very important to correctly evaluate the patient’s original data. If we are talking about breast augmentation, it is very important to choose the right size of the implant. The larger the implant, the more it goes beyond the natural size of the breast, the higher the risks of complications that may occur in the postoperative period. Both the surgical technique and the implants themselves are important: which manufacturer, which structure of implants. Of great importance is the way the body reacts to such implants, so you should focus on a high technical level, use the right technology, expensive implants that have proven themselves.
Blepharoplasty
Blepharoplasty is a very popular operation in the Russian market. In blepharoplasty, there are several approaches: either excess fat is removed or redistributed, or these methods are combined. The method is selected depending on the evidence and the competence of the doctor. Blepharoplasty can be considered unsuccessful when too much fat is removed and as a result we get the syndrome of the “empty” eye. Such sunken, "empty" eyes due to the fact that a lot of soft tissue is removed. This is one of the signs of old age, and it looks aesthetically not very attractive. A plastic surgeon must understand where that line is that cannot be stepped over, in order to make the aging eye even more age or even the sore eye, and the “empty” eye looks that way. The second, a most common complication is the reversal of the lower eyelid. Occurs when excessively aggressive removal of the skin and aggressive manipulations with the circular muscle of the eye are done. The circular muscle and lower eyelid lose their tone, the skin begins to pull down, the eyelid turns out. Another complication is also encountered: the incrustation of the incisions is incorrect, that is, visible postoperative scars. Very often I see that this operation is done without evidence. For example, young people and girls are undergoing such operations, I consider this to be unacceptable. At a young age, the removal of periorbital fat, which some surgeons consider superfluous, will eventually lead to very unattractive aesthetic consequences. To correct them, it is necessary, on the contrary, to do micro lipofilling, to transfer fat using cannulas to the upper eyelids, in order to recreate this young volume. Much depends on the chosen strategy. I recommend looking towards the trends of redistribution of fat packages when performing blepharoplasty, rather than their excision, or a competent combination of these methods. The last option, in my opinion, is most preferable.
